Services provided by Elderhaus at the Lake are divided into two programs; Adult Day Care and Adult Day Health. Adult Day Care is in large part a social and recreational program, safely arranging interactive group activities and exercises for seniors to enjoy with their peers.
Our Adult Day Care participants must be:
- 18 years of age or older
- Able to perform activities of daily living (eating, walking, transferring) with minimal assistance
- Ambulatory or Semi-Ambulatory and able to be safely transferred
- Have a completed medical exam report signed by a physician, nurse practitioner, or physician’s assistant completed within 3 months prior to enrolling
- Have no communicable disease documented on medical exam.
Seniors enrolled in Adult Day Health enjoy the same social and recreational opportunities but must have a medical need requiring the monitoring and services of a nurse. These participants usually require more assistance with activities of daily living than Adult Day Care participants.
To be eligible for our Adult Day Health program participants must be:
- 18 years of age or older
- Need assistance with activities of daily living (feeding, ambulating, toiling, etc.)
- Need health care monitoring by a registered nurse
- Need assistance with the administration of medications, special feedings, or other treatments
- Have a completed medical exam report signed by a physician, nurse practitioner, or physician’s assistant completed within 3 months prior to enrolling
- Have no communicable disease documented on medical exam.
Both Adult Day Care and Adult Day Health participants may require administration of medications, special feedings, or provision of other treatments as ordered by a physician. Each Adult Day Health participant has a plan of health designed and monitored by our Licensed Nurse.
All participants have individual service plans that outline their individual needs and interests. Program goals are designed to meet these individual needs.
Elderhaus at the Lake (Admission/Enrollment) Process
After determining eligibility, we will work with you to begin the following process for Enrollment:
- The applicant’s primary care physician must complete a Medical Examination Form.
- Schedule a visit or simply stop by and take a tour of our facility.
- Our Center Director will sit down with you to go over the enrollment packet and answer any questions you may have.
- You will then meet with our Health Care Coordinator for a short assessment of your medical needs.
- Finally, we will meet with you to complete your paperwork and establish your individual goals for care.
