Who Is Eligible?
You can join PACE if you meet the following conditions:
- You at least 55 years of age
- You are certified as having met the North Carolina requirements for nursing facility care
- You are able to live in the community without jeopardizing your health or safety
- You live in one of the approved zip codes
- You have a payment source (Medicaid, Medicare, Private Funds or Private Insurance)

How to Enroll
Enrollment in the PACE program is extensive to ensure we have the best possible assessment of your needs. The entire process generally takes 6-8 weeks from the initial home visit to establishing an individual plan of care. The steps are as follows:
- Our Intake Coordinator is available to meet with you in your home to answer any questions you may have and help you to assess your eligibility.
- You will schedule a visit to our Day Health Center where you will tour our facility, meet our care team, and begin the application process.
- After we have reviewed your medical record, members of our interdisciplinary team will visit your home to observe for safety.
- You will be scheduled for a second visit of our facility where our staff will complete their assessment of your medical needs.
- Our interdisciplinary team will meet with you to complete your enrollment packet and establish an individual plan of care.

Funding
If you have Medicaid, you do not have a monthly premium for the long-term care portion of the PACE benefit.
If you don’t qualify for Medicaid but you have Medicare, you’ll be charged a monthly premium to cover the long-term care portion of the PACE benefit and a premium for Medicare Part D drugs.
There is no deductible or copayment for any drug, service, or care approved by your health care team.
If you do not have Medicaid or Medicare, you can always pay for PACE privately.
“Lock-In” Provisions
As an Elderhaus PACE Participant, all health care services are provided and arranged by your IDT team. If you receive medical services without prior authorization (with the exception of emergency services), you may be liable for the full cost of those services.
